How to Become a Radiologist: Requirements, Degrees and Salary

Radiologists interpret imaging — X-ray, CT, MRI, ultrasound, PET — and interventional radiologists perform image-guided procedures. US median pay is about $380,000, and teleradiology has made it one of the most location-flexible specialties.
Degrees You Must Earn
- Bachelor's degree (4 years) with pre-med prerequisites; physics, computer science, biomedical engineering and biology are all strong majors, and coding skills are now a genuine advantage.
- MD or DO (4 years); Europe/Turkey: 6-year medicine faculty.
- Diagnostic radiology residency (5 years: 1 intern year + 4 radiology years), or integrated interventional radiology residency (6 years).
- Optional fellowship (1 year): neuroradiology, musculoskeletal, body imaging, breast, pediatric, nuclear medicine.
Admission Requirements
Competitive but attainable: Step 2 CK in the 245-255 range, some research, and demonstrated interest through imaging electives. Interventional radiology is significantly harder to match than diagnostic.
Licensing and Certification
USMLE Steps 1-3, state license, and American Board of Radiology certification — the CORE exam after 36 months of residency and the CERTIFYING exam 15 months after residency. Europe: national boards plus the European Diploma in Radiology (EDiR).
The AI Question
AI now handles triage, nodule detection and quantification, but reimbursement, liability and complex case synthesis keep radiologists central. The realistic 2026 view: radiologists who use AI tools read more studies per hour and earn more, while purely volume-based reading is under pressure. Learning imaging informatics is career insurance.
Realistic Timeline
18 → 22 bachelor's → 26 MD → 31 residency complete → 32 after fellowship.
Pay Progression
Resident $65,000-$80,000; first-year diagnostic attending $350,000-$420,000; private-practice partner $500,000-$700,000; interventional radiology $450,000-$600,000. Remote teleradiology night coverage pays a premium of 20-40%.
Skills That Decide Success
Pattern recognition, anatomical mastery, structured reporting discipline, comfort with high daily volume, and — increasingly — the ability to validate and audit AI outputs.
